Physician Progress Visits
Monthly visits tracking growth, BMI percentile, blood pressure, lifestyle progress, and goal review — led by Dr. Samuel Olaleye, MD & Medical Director.
Nutrition Coaching
Family meal planning, healthy snack education, school lunch guidance, restaurant strategies, hydration goals, and sugar reduction.
Sleep Optimization
Bedtime routines, sleep hygiene, screen time management, and restorative sleep habits — targeting 8–10 hours nightly.
Movement Planning
Individualized exercise recommendations based on age, interests, fitness level, and health goals — making movement feel natural.
Parent Partnership
Education and support for healthy home environments, grocery shopping, family meals, and positive reinforcement strategies.
Advanced Metabolic Evaluation
Laboratory testing when medically appropriate — HbA1c, fasting glucose, insulin, lipid panel, CMP, Vitamin D, and thyroid.
Bi-weekly GLP-1 therapy
for qualifying patients.
For patients who meet clinical criteria, the Elevate Program™ includes physician-prescribed GLP-1 receptor agonist therapy — administered bi-weekly via subcutaneous injection under physician supervision. Semaglutide is FDA-approved for adolescent obesity treatment.
Semaglutide
GLP-1 Receptor AgonistFDA-approved for chronic weight management in adolescents 12+ with obesity (BMI ≥95th percentile) or overweight (BMI ≥85th percentile) with at least one weight-related comorbidity. Dose titrated under physician supervision.
Ages 10–11
For patients ages 10–11, GLP-1 therapy is considered on a case-by-case basis under physician discretion when clinically appropriate. Eligibility is determined through the Initial Elevate Assessment, laboratory evaluation, and parental consultation with Dr. Samuel Olaleye, MD & Medical Director.
GLP-1 medication is prescribed only when clinically appropriate and is subject to physician evaluation, laboratory work, and parental consent. The bi-weekly semaglutide injection is included in the $150/month enrollment fee — no waiting, no separate pharmacy bill for your GLP-1. Insurance may be used for other prescriptions outside the GLP-1 scope — such as Zofran, Vitamin B12, and Vitamin D — and we will assist with prior authorization when applicable. Bi-weekly injection visits are performed in-clinic and included in the monthly enrollment fee.
